Application for Permission to Reprint
Please fill out the following information in the application form. Applications with incomplete or insufficient information may not be processed.

- Name of person in charge, company name, facility name, etc.
- Copyrighted material you wish to use (URL, media name, etc.), and the title of image, photo, diagram, etc.
- Please attach a copy of a proposed final document in which our contents are quoted or reprinted, such as a draft of the publication. (A separate file is acceptable.)
- Purpose of use (Please describe as much detail as possible, including the method of use, the type of media, and the method of publication).
Flow of the Application
- Please make sure to fill out all form fields of Application for Permission to Reprint.
- Please submit the Application for Permission to Reprint to the contact below(please scan and email the PDF file).
- We will review your application based on the purpose of use and the drafts. It may take more than one week depending on the contents.
- We will inform you later of the result of your application.
- Please send us a copy of your final publication. We would appreciate your cooperation in this regard. By establishing and utilizing an international trial network, we promote the development and introduction of new medical products and contribute to the improvement of medical care.
